  1. Distribute Sheets 3a and 3b to patients or individuals collecting patient information.
  2. Retrain any participating person completing the sheet 3a and 3b as necessary
  3. Provide  tools, such as pencils etc.
  4. Collect sheets 3a and 3b after  afternoon snack patient snack of the day
