  2. Discuss this trial with involved staff and clinicians to ensure that everyone understands the nutritionDay processes
  3. Access the nutritionDay website, www.nutritionday.org
  4. Click on questionnaires
  5. Print the following documents: Patient Consent Form, Sheet 1a, 1b, 1c,, Sheet 2, Sheet 3, Sheet 4, Patient Non Consent Form (found under Hospitals & ICU)
