Inform local managers, key clinicians and other clinical nutrition team members and apply for Internal Review Board (IRB) approval if required. This may just require an IRB waiver, expedited process or exemption.
nutritionDay in the U.S. is an “audit” or “registry”. Therefore considered a minimally invasive study. Patient care does not change on nutritionDay. An IRB review can be timely. So we recommend you start this process promptly.
Also, inform you IRB that nutritionDay is annual program.
STEP I: Register for nutritionDay
- Skip this step if you already registered from prior nutritionDay participation
- Access the global (www.nutritionDay.org) or U.S. (www.nutritionDayUS.org) website, click on “register”
- Complete the required information
- Receive a confirmation email within 24 hours.
- Respond, as requested, to the confirmation email
- You will need your username and password to register for your facility and unit codes, your next step.
- Skip this step if you already have a facility code from prior nutritionDay participation
- Access the global nutritionDay website: www.nutritionday.org
- Click on “order code” on top left side of site
- Enter user name and password from your registration process (see step 1, above)
- Welcome to nutritionDay screen will appear
- Click on “to order new codes click here” this is located on the bottom of the text.
- Click on “order new center code”
- Complete the basic facility profile information requested
- Click on “order a center code”
- An overview of your registered facility and the associated unit code will appear.
- You can order a additional codes via the same process if necessary
- Within 24 hours, you will receive an email confirming your facility code.
- “ordered not yet active” will become “active” once your facility code has been confirmed by responding to the email described above.
STEP III: Order Participating Unit Code(s).
- Skip this step if this unit was registered for nutritionDay in previous years.
- Access the global nutritionDay website: www.nutritionday.org
- Click on “order code” on top left side of site
- Enter user name and password from your registration process (see step 1, above)
- Welcome to nutritionDay screen will appear
- Click on “to order new codes click here” this is located on the bottom of the text.
- Click on the centercode’s name in orange to access the unit code order form
- Complete the brief unit questionnaire
- Click on “order a unit code”
- An overview of your registered center and the associated unit code is shown.
- To order another unit code, scroll down the form
- Repeat steps 1-5.
- Continue steps 10 and 11 above for all of the participating units.
- You will receive a confirmation email within 24 hours after each unit code is processed.
- Please confirm the unit code by responding to this email
- Within 24 hours, “ordered, not yet active” will change to “active” after your code has been confirmed.
- You can always order more codes, if necessary
One Month Before nutritionDay
STEP IV: Conduct a nutritionDay Trial for 1 Patient
- Designate a team lead for each unit participating in nutritionDay.
- Discuss this trial with involved staff and clinicians to ensure that everyone understands the nutritionDay processes
- Access the nutritionDay website, www.nutritionday.org
- Click on questionnaires
- Print the following documents: Patient Consent form, Sheet 1, Sheet 2, Sheet 3a and Sheet 3b
One Week Before nutritionDay
STEP V: Prepare Resources for nutritionDay
- Complete Sheet 1/Unit Sheet
- Refer to Frequently Asked Questions (FAQs), under “Information Finder” than “Help” to assist with completing the nutritionDay forms.
- Print adequate number of questionnaires and patient consent forms for the participating patients
- Train all involved staff on the completion process for these forms.
- Assign roles and responsibilities for nutritionDay. Include distributing and collecting questionnaires, and patient/family member training
- Ensure that all the necessary supplies are available, such as pencils, folders, etc.
One Day Before nutritionDay
STEP VI: Complete Sheet 2/Patient List
- Available on nutritionday.org, see STEP IV
- Click questionnaires
- Click on sheet 2 in English
- Provide participating patient information
- Assign a code for each participating patient
STEP VII: Receive Patient Consent, if necessary
- Use available nutritionDay or facility provided patient consent forms
- Receive support from relatives/friends or patients to complete questionnaires
- Please encourage your patients to participate as appropriate.
On nutritionDay
STEP VIII: Data Collection
- Distribute Sheets 3a and 3b to patients or individuals collecting patient information.
- Retrain any participating person completing the sheet 3a and 3b as necessary
- Provide tools, such as pencils etc.
- Collect sheets 3a and 3b after afternoon snack patient snack of the day
After nutritionDay
STEP IX: Enter Data
- Have Sheets 1, 2, 3a, and 3b available
- Access the nutritionDay website, www.nutritionday.org
- Click on “data”
- Under electronic data transfer select your institution
- Click on “log in”
- Enter your data as requested.
30/60/180 Days After nutritionDay
Step X: Outcome Data Collect and Enter
NON-ICU: 30 Days after nutritionDay data is collected
ICU: 60 Days after nutritionDay data is collected
180 Days or 6 months: Long Term Care
- Through a medical record review, complete the Unit Patient list outcome for all participating nutritionDay patients.
- Submit this data electronically within 30 days after your designated outcome data data described above on www.nutritionday.org. For detailed instructions, use the following link: http://www.nutritionday.org/uploads/media/guidance_for_data_entry_automation_2011_.pdf
- Follow the same steps for entering in your nutritionDay data.
Obtain your report
non-ICU and Long Term Care: Upon completion of your outcome data entry,
- Click on feed back report.
- Download Feedback Report
- Correct all missing information described on the feedback report.
- Check Feedback Report and acceptance
- Download preliminary report
ICU
Email from office@nutritionDay.org will be sent to registered nutritionDay leader informing you of your report availability and will provide download instructions.
